Every caregiver is an employee of Hi-Pointe Care Solutions. To help ensure the safety and well being of our clients, we perform a criminal history background clearance and then cover each caregiver with general liability and workers’ compensation insurance.

Yes, Hi-Pointe Care Solutions will partner with you to teach and assist you with care activities of your loved one and monitor their progress. Hi-Pointe will work with the physician and prepare you to care for loved one until you can safely care for them alone.

  • How will I know when it is time for my loved one to be discharged from home care?

Progress and goals will be reviewed by the Registered Nurse and/or the Clinical Manager on regular a basis. Once the goals have been met and a safe discharge plan has been determined, the physician will provide the discharge order to the agency staff. The nurse will notify you and again review and answer questions prior to the time and date of the discharge. We will also assist you in identifying other resources that may be needed.

  • How is the privacy of my personal information maintained?

We adhere to all applicable federal and state guidelines related to privacy of personal and healthcare information. This starts once we have a formal services agreement, even before a caregiver comes to your family member’s home. The written service agreement stipulates the exact restrictions on the use of your information, which clarifies that even unauthorized individuals will not be given access to information without a client’s – or legally authorized representative’s – formal consent.

The physician, hospital discharge planner/case manager, and or the nurse.

You can make a self referral on behalf of your loved one to the Hi-Pointe. You can call your physician, case manager and or social worker. Services MUST be ordered by your physician.

Contact your insurance company and ask if home care is a covered benefit for your loved one.

Hi-Pointe specializes in caring in caring for patients of all ages.

Business Hours are: 9:00 a.m. to 5:00 p.m. Monday through Friday. However, an administrator and RN is on call 24 hours a day, 7 days a week.

 

  • If my insurance will not cover the service but I wish to pay for services that Hi-Pointe provides, will Hi-Pointe accept private pay?

Yes; consideration is made on a case –by- case basis and will depend on the type of service requested, and the availability of qualified staff.
