To meet the needs of clients with significant medical conditions, Hi-Pointe Care Solutions offers skilled nursing services for patients of all ages. Performed by Registered Nurses (RNs) or Licensed Practical Nurses (LPNs), these services often include advanced care coordination, initial evaluation, and creation and oversight of short-term and long-term care plans for our clients. When the elderly, injured, or sick can be cared for in the comforts of their own home instead of a hospital or other medical facility, it can help ease their stress.
Our nursing staff also provides expert supervision and training to the rest of our caregivers, including Certified Nursing Assistants (CNAs), personal care aides, home health aides, companies, and other individuals involved in the care of our clients. Under the supervision of a nurse, our caregivers can also provide vital sign monitoring.
Because of their professional skills and training, our nurses can make healthcare referrals or recommendations to maintain or improve the well-being of our clients. This may include appointments with physical or occupational therapists, installation of ramps, or use of durable medical equipment or oxygen supplementation. Nurses may also be able to re-order and pick up medications, set up a medication regimen, and track adherence to the recommended schedule.
Some clients benefit from a nurse accompanying them to visit their physicians or other healthcare practitioners. Our RNs, LPNs, and some exceptionally skilled CNAs are often able to relay information regarding the client’s condition, help our clients and their family understand and execute follow-up care and instructions, and manage a schedule of appointments.
Our nurses may also provide nutritional assistance such as planning menus, creating a grocery list, preparing meals within nutritional guidelines, educating clients about their diet, and tracking clients’ food intake. Families often find these types of solutions beneficial to care.
